Step on a scale, calculate your BMI, land in the 'normal' range, and it's natural to assume your weight isn't a health worry. For a lot of people, that assumption is reasonable. Among people whose BMI was in the normal or overweight range, having a clinically high waist measurement or waist-to-hip ratio was associated with roughly 15 to 50 percent higher risk across most of those nine outcomes [1, 2]. In other words, you could have a perfectly normal number on the scale and still carry a substantially elevated risk to your heart, one that BMI completely missed. The study's authors concluded plainly that relying solely on BMI can misclassify cardiovascular risk, and JACC's editor-in-chief called it time to 'abandon a sole focus on body mass index' [2, 3].
The reverse held too: some people with a BMI in the obese range but low abdominal fat didn't show the same elevated risk [3]. The lesson isn't that weight doesn't matter. It's that fat distribution, not total weight, is what drives much of the cardiovascular danger, and BMI is blind to distribution.
